Your LDL trajectory tells a story. Make sure it's being told.

A single cholesterol panel tells you where you are today. A series of panels over months and years tells you whether treatment is working, whether your lifestyle changes are making a difference, and whether your cardiovascular risk is trending up or down.

This log was designed around the full lipid panel Dr. Nyange reviews — not just total cholesterol, but LDL, HDL, non-HDL, triglycerides, and increasingly important markers like Lp(a) and ApoB. It also captures what medication you were taking at each measurement — so you can see exactly how your numbers respond to dose changes.

For patients on statins, this log is invaluable: it shows what your LDL was before starting, how much it dropped, and whether you're hitting your personalized target.

"Cholesterol management is a long game. I want to see where a patient started, what medications they've tried, how their numbers responded to each change, and whether we're consistently hitting their LDL target. That history — captured in one organized document — makes every follow-up appointment far more productive."

Common Questions

What LDL goal should I be targeting?
LDL goals are individualized based on your cardiovascular risk. Very high-risk patients (prior heart attack, known coronary artery disease) typically target LDL below 55–70 mg/dL. High-risk patients target below 70 mg/dL. Dr. Nyange will set your specific goal at your consultation.
What is Lp(a) and should I test for it?
Lipoprotein(a) is a genetic lipid particle that significantly elevates cardiovascular risk in people with elevated levels. The AHA recommends at least one Lp(a) measurement in all adults. This log includes a field for it — ask Dr. Nyange about testing if you haven't already.
How often should I get my lipids checked?
For patients on statin therapy, Dr. Nyange typically recommends a fasting lipid panel 4–12 weeks after starting or changing medication, then every 3–12 months depending on stability. Ask at your next appointment.
Can I track my spouse's or family member's cholesterol here too?
The file is built for one patient. For a family member, simply download a second copy and use it independently.
